After some of the experts looked at it in detail, we decided to use a different attribute called "DN Qualifier" whose X.500 definition is: "The DN Qualifier attribute type specifies disambiguating information to add to the relative distinguished name of an entry. It is intended to be used for entries held in multiple DSAs which would otherwise have the same name, and that its value be the same in a given DSA for all entries to which this information has been added." So it fits perfectly for the intended purpose. Also, it was not very clear, but using the NPI or PayerID in the DN, although possible, would not serve as disambiguating for providers/payers that have more than one cert. So, if the NPI, UPIN, PayerID, EIN, etc. are to be listed in the cert, they will not be part of the DN, but will be part of the cert and/or the directory entry. The disambiguating DN Qualifier must be capable of distinguishing that certificate from other certificates held by the same subject.
